VALID IDENTIFICATION CARD OF THE APPLICANT (Photo Copy Front & Back) such as, but not limited to the following, to show proof that he/she is the applicant, e.g.: Passport, Driver’s License, PRC License, Company ID, SSS, GSIS, TIN, Philhealth, Senior Citizen ID or Postal ID
BARANGAY CERTIFICATE/CLEARANCE (Original Copy) with the specific purpose: FOR ELECTRIC SERVICE CONNECTION,
AFFIDAVIT OF WAIVER (Original Copy) from the City/Municipal Engineer’s Office if lot is owned by the government, or,
MEMORANDUM OF UNDERTAKING (Accomplished/Original Copy) if lot is owned by a private individual;
VALID IDENTIFICATION CARD OF THE LOT OWNER (Photo Copy Front & Back) such as, but not limited to the following, to show proof that he/she is the applicant, e.g.: Passport, Driver’s License, PRC License, Company ID, SSS, GSIS, TIN, Philhealth, Senior Citizen ID or Postal ID
LEASE CONTRACT (Photo Copy) For Commercial and Industrial Applicant
PROOF OF OWNERSHIP (Photo Copy) if lot is owned by the applicant, e.g. TAX DECLARATION, TCT, and DEED OF SALE;
LIVE BIRTH / BAPTISMAL CERTIFICATE (Photo Copy),
ELECTRICAL PLAN (Original Copy) Blue Print or PEE Form, sealed by the Professional Electrical Engineer
FIRE SAFETY INSPECTION CERTIFICATE or ELECTRICAL PERMIT issued by the Bureau of Fire Protection
AFFIDAVIT OF TRANSFER (Original Copy) For Transfer of Connection
PMES Certificate
